Cancellation Policy / Late Policy / No Show Policy

You may reschedule or cancel up to 24 hours before the start time of your appointment.
Work or School Physicals, DOT Exams, or USCG Physicals must arrive 20 minutes prior to appointment time to complete the required paperwork. All other appointments must arrive 10 minutes early.
If you realize you will be late, please call us as soon as possible. We will do our best to accommodate you. This may include helping you to reschedule. Any late arrivals, changes, or cancellations to a scheduled appointment within 24 hours of the start time will result in a fee of as listed on the “fees” page of the website. https://www.latitudeclinic.com/fees/
